Care Coordinators working for SeniorsPlus in the State of Maine assist clients to make informed choices to live safely and independently in their communities.
Responsibilities
• Coordinates care adhering to a philosophy of person centered, strength based, proactive planning while using a collaborative partnership approach and follows his/her applicable practice and ethical standards
• Completes psychosocial assessments, risk assessment, goal development, action plans, and back-up plans
• Adheres to contract requirements and current state program regulations in accomplishment of daily work
• Upholds the values of client respect, choice, access, dignity, quality, prevention and early detection
• Works with providers to gather information about Medicare or services supported by third party Insurers
• Ensures evaluation of the effectiveness of the plan of care, makes recommendations for care plan modifications and identifies barriers
• Adheres to all appropriate protocol related to face to face visits (home visits), including documentation and appropriate and timely communication for follow up
Qualifications
• Bachelor’s degree in Health or Social Services required
• Minimum of one year in Social/Human/Community Services required
• Familiarity with the long term care system a plus, strong computer and organizational skills required
