ESM, Inc., Maine's premier social services agency, is seeking applicants for the position of “ID Community Case Manager” within ESM's Western Division in Auburn. This key position assists individuals with cognitive disabilities in finding resources to help support them in day-to-day life and become successful community members. $3,000 Sign-On Bonus!
The primary role of an ID Community Case Manager is to assist in identifying and implementing support strategies that reflect the client's personal vision. The Case Manager works with the client and other individuals that the client identifies such as family, friends, and service providers in developing a person-centered plan and then assists with follow through on goals and objectives. The Case Manager will monitor services and work with the client to assure his/her satisfaction with the process and outcomes of support, services and available resources.

Requirements A valid driver's license, safe driving record, reliable transportation and vehicle insurance that meets Maine legal requirements. ESM employees must be caring, show enthusiasm, and have a good work ethic.
Benefits ESM offers weekly pay, Health, Vision, and Dental Insurance (100% paid by employer), and Allstate insurances (100% paid by employee), Paid Time Off (PTO) to use in the first year of service and a set time amount annually that increases with years of service, nine (9) paid holidays, mileage reimbursement, and 401K fund after eligibility is met (25% employer match up to $20.00 per week).
Education The successful applicant will have a Bachelor’s Degree in the social services field with 2+ years of Human Service experience.
